Personal information protection policy
Staffmed Solutions Inc. is committed to safeguarding the personal information entrusted
to us by our Clients and our employees. We manage your personal information in accordance
with Ontario Information Protection Act and other applicable laws. The policy outlines
the principles and practices we follow in protecting your personal information.
The principles and practices in protecting your personal information include: Collection
personal information: individual’s name, home address, phone number, age, financial
information, educational history, etc. Personal information is collected for the
purpose provide our Clients with name, background individual who provides services,
payroll company ADP Canada to process employees’ payments, Revenue Canada with identification
employee with employee ID number, calculate gross amounts, stat; deductions and
remittances and T4 forms.
Consent: We ask for consent to collect, use or disclose employee/Client personal
information to ADP Canada, government agencies, Revenue Canada, WSIB, Ministry of
Labour, Law offices. We do not disclose any employee/Client information to any other
organization or/and individual for the purposes of references, employee performance
without written consent of individual who’s personal information is requested.
Personal information being collected, use and disclosed
We collect, use and disclose personal employee information to meet the following
purposes:
- Determine eligibility for employment incl. verifying qualification and references
- Establishing training and development requirements
- Assessing performance and managing performance issues if they arise
- Administering pay and benefits (pay employee’s only)
- Complying with applicable laws (Canada income TAX Act, Employee Standards Code)
Personal information we might provide for employment/references
In some case after/during your employment we will be contacted by other organization
and asked to provide references. It is our policy not to disclose personal information
about our employees to other organization without writer consent of the employee.
We might provide references included: -confirmation of employment including position,
starting date, hourly wages -general information about employee duties, performance.
Safeguard and secure personal information
We make every reasonable effort to ensure that personal information is accurate
and complete. We rely on individuals to notify us if there is a change to their
personal information that may affect their relationship with our organization. Employee
files with personal information are stored in safe cabinets with locker. The only
authorized person has access to employees’ files. The electronic files are secured
by encrypted password and every coordinator has assigned log in.
Access to personal/Client information
We grant you access to your file in case you need to make correction, change or
add your personal information. We refuse to provide with personal information of
our employee another employee and/or Client in order to contact the person and/or
discuss issues. We decline request of Client to reveal employee phone number in
order to clarify work related information We refuse to our Client to contact our
employee directly for any purpose as per Personal Information Protection Act and
suggest to our Client that employee will contact the Client as soon as possible.
Confidentiality statement about company’s /Client’s information.
We keep confidential Client’s information about residents, diagnosis, treatments.
We have access to Client’s documents when we work on Client premises. We have access
to Client’s policies, procedures and regulations. We will not disclosed the information
to other Clients or/and discuss about confidential information with third party.
We respect Client confidentiality policy and follow them.
